Add logging in route hint filtering
Knowing why a channel was not included as an invoice route hint can be
valuable. Add logging on the decisions made when filtering channels.

c7c88a6572 Export and document all log macros.
Previously, only `log_error` and `log_trace` macros have been exported.
This change exports the macros of all log levels, which enables them to
be used downstream.

7fa6a7d48e Allow logging to specify an explicit log level instead of a macro
For log entries which may have a variable level, we can't call an
arbitrary macro and need to be able to pass an explicit level. This
does so without breaking the compile-time disabling of certain log
levels.

Further, we "fix" the comparison order of log levels to make more
significant levels sort "higher", which implicitly makes more sense
than sorting "lower".

Finally, we remove the "Off" log level as no log entry should ever
be logged at the "Off" level - that would be nonsensical.

Move ChannelMonitor state behind a Mutex
ChainMonitor accesses a set of ChannelMonitors behind a single Mutex.
As a result, update_channel operations cannot be parallelized. It also
requires using a RefCell around a ChannelMonitor when implementing
chain::Listen.

Moving the Mutex into ChannelMonitor avoids these problems and aligns it
better with other interfaces. Note, however, that get_funding_txo and
get_outputs_to_watch now clone the underlying data rather than returning
references.

a21decf0db Struct-ify SpendableOutputDescriptor entries relevant to channels
Both SpendableOutputDescriptor::DynamicOutputP2WSH and
SpendableOutputDescriptor::StaticOutputCounterpartyPayment are
relevant only in the context of a given channel, making them
candidates for being passed into helper functions in
`InMemoryChannelKeys`. This moves them into their own structs so
that they can later be used standalone.

6df9129ace Use ln OutPoints not bitcoin ones in SpendableOutputDescriptors
Lightning OutPoints only have 16 bits to express the output index
instead of Bitcoin's 32 bits, implying that some outputs are
possibly not expressible as lightning OutPoints. However, such
OutPoints can never be hit within the lightning protocol, and must
be on-chain spam sent by a third party wishing to donate us money.
Still, in order to do so, the third party would need to fill nearly
an entire block with garbage, so this case should be relatively
safe.

A new comment in channelmonitor explains the reasoning a bit
further.

4395b92cc8 Relicense as dual Apache-2.0 + MIT
This changes the LICENSE file and adds license headers to most files
to relicense under dual Apache-2.0 and MIT. This is helpful in that
we retain the patent grant issued under Apache-2.0-licensed work,
avoiding some sticky patent issues, while still allowing users who
are more comfortable with the simpler MIT license to use that.

8ade071d56 Drop OutPoint::new since the struct is all pub
This makes it easier for our automated bindings generator to
function as it tries to automatically create a ::new if the struct
contains only pub elements who's type is convertible.

ChannelManager+Router++ Logger Arc --> Deref
This caused a bunch of cascading changes, including
passing loggers down to Channels in function calls
rather than having each Channel have a pointer to the
ChannelManager's Logger (which was a circular reference).
Other structs that the Channel had passed its Logger to also
had their loggers removed. Other newly unused Loggers were
also removed, especially when keeping them would've caused
a bunch of extra test changes to be necessary, e.g. with
the ChainWatchInterfaceUtil's Logger.

c89514c37c De-Option<> some fields in ChannelMonitor which are set at init
After we moved the ChannelMonitor creation later during Channel
init, we never went back and cleaned up ChannelMonitor to remove
a number of now-useless Option<>s, so we do that now.

fbc7885a97 Bound incoming HTLC witnessScript to min/max limits
Fix a crash where previously we weren't able to detect any accepted
HTLC if its witness-encoded cltv expiry was different from expected
ACCEPTED_HTLC_SCRIPT_WEIGHT. This should work for any cltv expiry
included between 0 and 16777216 on mainnet, testnet and regtest.

1a870fe8aa Fix crash when a claim tx has some non-witness inputs.
The logger which decides what to refer to an on-chain claim tx was
assuming that all inputs would have a witness. While this was fine
for the one-input case, it broke the fuzzer which was connecting a
consensus-invalid transaction. Further, in the case we have multiple
inputs, some may not have a witness, which we shouldn't crash on.

This fixes 9df0250dbb.
