contractcourt: fix concurrent access to resolved
This commit makes `resolved` an atomic bool to avoid data race. This
field is now defined in `contractResolverKit` to avoid code duplication.

contractcourt: add Launch method to anchor/breach resolver
We will use this and its following commits to break the original
`Resolve` methods into two parts - the first part is moved to a new
method `Launch`, which handles sending a sweep request to the sweeper.
The second part remains in `Resolve`, which is mainly waiting for a
spending tx.

Breach resolver currently doesn't do anything in its `Launch` since the
sweeping of justice outputs are not handled by the sweeper yet.

contractcourt: add verbose logging in resolvers
We now put the outpoint in the resolvers's logging so it's easier to
debug.

contractcourt: remove the immediate param used in Resolve
This `immediate` flag was added as a hack so during a restart, the
pending resolvers would offer the inputs to the sweeper and ask it to
sweep them immediately. This is no longer need due to `blockbeat`, as
now during restart, a block is always sent to all subsystems via the
flow `ChainArb` -> `ChannelArb` -> resolvers -> sweeper. Thus, when
there are pending inputs offered, they will be processed by the sweeper
immediately.

This commit makes sure the time-sensitive outputs are swept immediately
during startup.

sweep: don't give up an input based on number of attempts
This commit removes the logic where we remove an input when it's been
published more than 10 times. This is needed as in our future fee
bumper, we might start with a low fee and rebroadcast the same input for
hundred of blocks.

contractcourt: handle sweeping script-enforced leased channel outputs
In order to sweep the commitment and HTLC outputs belonging to a
script-enforced leased channel, each resolver must know whether the
additional CLTV clause on the channel initiator applies to them. To do
so, we retrieve the historical channel state stored within the database
and supplement it to the resolvers to provide them with what's needed in
order to sweep the necessary outputs and resolve their respective
contracts.

cnct: cpfp-sweep anchors
For unconfirmed commit tx anchors, supply the sweeper with cpfp info and
a confirmation target fee estimate.

The sweeper will try to pay for the parent commit tx as long as the
current fee estimate exceeds the pre-signed commit tx fee rate.

sweep: make sweeper aware of unconfirmed parent transactions.
Extend the fee estimator to take into account parent transactions with
their weights and fees.

Do not try to cpfp parent transactions that have a higher fee rate than
the sweep tx fee rate.

cnct: clear exclusive group on anchor sweep after confirmation
The sweeper call UpdateParams does not update the exclusive group
property of a pending sweep. This led to anchor outputs being swept
after confirmation with an exclusive group restriction, which is not
necessary.

This commit changes the anchor resolver to not use UpdateParams anymore,
but instead always re-offer the anchor input to the sweeper. The sweeper
is modified so that a re-offering also updates the sweep parameters.

cnct: resolve anchors post-confirmation
Sweeping anchors and being able to bump the fee was already added in a
previous commit. This commit extends anchor sweeping with an anchor
resolver object that becomes active after the commitment tx confirms.
At that point, the anchors do not serve the purpose of getting the
commitment tranaction confirmed anymore. It is however still possible to
reclaim some of their value if using a low fee rate.
