There are hardly any lightningd-specific JSON functions: all that's left
are the feerate ones, and there's already a comment that we should have
a lightningd/feerate.h.

We have them split over common/param.c, common/json.c,
common/json_helpers.c, common/json_tok.c and common/json_stream.c.
Change that to:
* common/json_parse (all the json_to_xxx routines)
* common/json_parse_simple (simplest the json parsing routines, for cli too)
* common/json_stream (all the json_add_xxx routines)
* common/json_param (all the param and param_xxx routines)

There is no "wallet_lib_headers" variable in wallet/Makefile
Likewise, there were two "lightningd_headers", a couple of unused
variables and some other nonsene in lightningd/Makefile
Currently it will be used for onion replies, but we can use it for offers
and invoices in future, if we want to avoid revealing our node_id.

This avoids duplication of both logic and error-prone values, such as
the salt. Grouping all hsm encryption logic into a public API will also
allow us to fuzz it.

"make clean" removes all object files listed in $(ALL_OBJS), which is derived from $(ALL_C_SOURCES).
Source files in $(LIGHTNINGD_SRC_NOHDR) are missing from $(ALL_C_SOURCES) therefore some object files are not removed by "make clean".
This commit fixes this point.

There's a few structs/wire calls that only exist under experimental features.
These were in a common file that was shared/used a bunch of places but
this causes problems. Here we move one of the problematic methods back
into `openingd`, as it's only used locally and then isolate the
references to the `witness_stack` in a new `common/psbt_internal` file.
This lets us remove the iff EXP_FEATURES inclusion switches in most of
the Makefiles.
I got a corrupt file, which looked like multiple concurrent attempts
to build it. So instead, build it in one command, but also use
VERBOSE so we print correctly with V=1 (and --quiet).
Also move into plugins/ where it logically belongs.

We're going to want this for bolt13 formation as well.
As a result of reworking the logic into "candidate selection" then
"route hint selection", we need to change the way round-robin works.
We use a simple incrementing index now.

This removes the reservation cleanup at startup, too, now they're all
using 'reserved_til'.
This changes test_withdraw, since it asserted that outputs were marked
spent as soon as we broadcast a transaction: now they're reserved until
it's mined. Similarly, test_addfunds_from_block assumed we'd see funds
as soon as we broadcast the tx.

We create ALL_PROGRAMS, ALL_TEST_PROGRAMS, ALL_C_SOURCES and
ALL_C_HEADERS. Then the toplevel Makefile knows which are
autogenerated (by wildcard), so it can have all the rules to clean
them or check the source as necessary.

Note that other directories were explicitly depending on the generated
file, instead of relying on their (already existing) dependency on
$(LIGHTNINGD_HSM_CLIENT_OBJS), so we remove that.

Since we now over-write the wally malloc/free functions, we need to do
so for tests as well. Here we pull up all of the common setup/teardown
logic into a separate place, and update the tests that use libwally to
use the new common_setup core

Adds a new plugin notification for getting information about coin
movements. Also includes two 'helper' notification methods that can be
called from within lightningd. Separated from the 'common' set because
the lightningd struct is required to finalize the blockheight etc

Note that it's channeld which calculates the shared secret, too. This
minimizes the work that lightningd has to do, at cost of passing this
through.
We also don't yet save the blinding field(s) to the database.

common/onion is going to need to use this for the case where it finds a blinding
seed inside the TLV. But how it does ecdh is daemon-specific.
We already had this problem for devtools/gossipwith, which supplied a
special hsm_do_ecdh(). This just makes it more general.
So we create a generic ecdh() interface, with a specific implementation
which subdaemons and lightningd can use.
